Southwest Nebraska Public Health Department (SWNPHD) completed a community health assessment and improvement plan in 2023 that described the current health status of their community, identified, and prioritized health issues, and identified assets and resources that could be mobilized to improve the health of the community.

The Community Health Assessment (CHA) is a systematic, data-driven approach to determining the health status, behaviors and needs of the residents of the nine counties SWNPHD services. This information may be used to inform decisions and guide efforts to improve community health and wellness. Our key findings are laid out in the infographic below.

The Community Health Improvement Plain (CHIP) is developed from the Community Health Assessment (CHA). In collaboration with community partners and key stakeholders, priority goals were identified to address priority areas of need.
